Hr Closing Date:

06/15/2026 Responsibilities Implementation of programs and agency policies and procedures Participates in the service plan process including creating initial IRP, crisis plans, risk assessments, as designated and in collaboration with director and clinician as needed. Attend team meetings as designated by the Director Provides rehabilitation, basic support, case management, and crisis interventions per member service plans Provides required documentation of services within the deadlines of the Agency Reports on residential status to Program Director and staff. Provides input to team decisions regarding admissions, discharges, and counselor assignments. Assists members in maintaining apartments and landlord standards when applicable. Assists members in financial matters as needed. Provides transportation related to services when necessary Maintains

CPR/FA/AED

certifications Monitors member compliance to their conditional release order when applicable. Required to provide coverage as needed at any of the Agency sites per Agency need Engages in continuous concerted efforts to access, obtain, respond to pertinent agency information/communications and disseminate as appropriate Provides conditions that maximize safety for co-workers and program participants. Other responsibilities of Reliance Health staff, such as participation in Agency events, adhering to Agency Policies, training requirements etc. Performs duties while demonstrating Agency Core Competencies and values Requirements Bachelor's degree in human services or equivalent work experience in the provision of mental health service Work experience in the provision of mental health services preferred Valid CT Driver's License, vehicle registration, and insurance Good driving record

$19.28 per hour
