Full Job Description Position Summary:
The Housing Case Manager position at Apex Community Care Inc.is a full-time, hourly position. The ideal candidate will be creative, compassionate and dedicated to supporting, empowering the clients in the program. The Housing Case Manager is responsible for all client's housing program requirements, including implementation of their care plans, processing all required paperwork for their housing needs, working with any Ryan White client who has housing needs/concerns within Apex's client roster. The Housing Case Manager responds effectively to the needs of all housing clients.
Responsibilities:
Provide Case Management to up to 25 HIV Program housing clients. Create and monitor care plans for clients enrolled in the housing programs and all necessary reporting needs. Monitor client housing needs and empower clients identifying and solving problems, adapting to changes in community needs. Help with processing payments as necessary for all funded housing services. Enter required information on HMIS as prerequisite by funding sources. Complete client satisfaction surveys. Develop and maintain working relationships with landlords, act as a liaison between landlord and client to locate and secure housing. Participate in bi-weekly house meetings and compile/ record requests from residents related to house needs. Oversee record keeping related to collection of house monies (i.e. program and house fee, phone bill, petty cash, etc.). Collaborate with local partner agencies and advocate for housing opportunities for PLWH in the greater Danbury area. Collaborate with statewide housing partners to ensure compliance with program requirements and share information and data within agency. Maintain working knowledge of housing program resources and eligibility requirements. Provide input and updates for the housing programs, policies, and procedures. Attend agency staff meetings and training. Perform other duties as assigned.
Qualifications:
Bachelor's Degree in Human Services or a related field Previous experience in program management and supervision Mental Health and Substance Abuse experience Reliability and flexibility Valid driver's license Strong interpersonal and leadership skills Strong written and oral communication skills Strong organizational skills and problem-solving skills At Apex Community Care, we value diversity and the uniqueness of all people.
