CMHA is looking to hire a Full-time, 2nd shift Case Manager for our Transitional Living Center (TLC) program. This position will be located at 66 Clinic Dr. New Britain, CT 06051. Sunday - Thursday, 3:30PM - 11:30PM.Compensation Range:
The salary range for this position starts at $20 an hour. Compensation for each position is determined not only by years of relevant experience, education, and skills, but also by maintaining internal pay equity. The Case Manager will be responsible for engaging and linking current or potential homeless clients with mental health or co-occurring disorders to MAT services. Serves as a liaison between CMHA and identified community providers to coordinate various aspects of care, including clinical services, housing support and individualized case management, as needed, to complement current services. Assist clients to ensure stable community living.Essential Responsibilities:
Provides advocacy, support and guidance with contacting external organizations (Social Security, DSS, DCF, DMHAS, etc). Educates clients to utilize emergency and community resources to help meet basic needs. Works with clients to teach life skills necessary to live independently in the community. This includes assisting with finances and entitlements, supporting the development of activities of daily living, nutrition, housing maintenance and repair, and prompting to encourage task completion. Collaborates and integrates with other CMHA teams to ensure all services provided to client are communicated with clients whole treatment team Assures timely documentation is maintained in accordance with CMHA policies and procedures, including but not limited to progress notes, treatment plans, authorizations, program orientation, and consent to treat, activity logs, and monthly targeted case management notes. Must have valid Connecticut driver's license, a reliable vehicle Will become certified in MANDT, First Aid, and CPR. Participate in trainings on an annual basis, as assigned.Experience :
Successful experience working in Case Management or related field with a solid understanding of Mental Health and co-occurring conditions, preferably a minimum of one year. Beginner to intermediate experience with Microsoft Office products including Word, Outlook, Excel and PowerPoint Experience in or the ability and willingness to learn how to use an Electronic Medical Record (EMR) system including: appointment scheduling, authorization tracking, assessments, treatment plans, progress notes and discharge summaries.Competencies :
Behavioral Skill Sets Ability to assess and formulate case management needs as expressed by the client and document in treatment plans. Effective interpersonal and communication skills with individuals with mental health and co-occurring disorders as well as with their families, caregivers and natural supports. Capability to problem solve and use good judgement, work independently and as a team member. Capacity to understand and incorporate recovery-based treatment, language, and interventions into documentation and daily operations. Work a flexible schedule when needed. Respond effectively to emergency situations. Design, deliver, and ensure highly individualized services and support. Ability to work in fast-paced environment, utilizing excellent time management skills, flexibility regarding scheduling, and the ability to balance a number of priorities based on workload in the completion of job responsibilities.
