Expected Starting Hourly Range:
$21.92•$27.40 Employees in this class perform professional tasks associated with social work and contract management activities in a variety of Aging Services Programs. Employees will work interdependently with community service provider agencies and customers to coordinate programs that help customers remain independent, and that ensure that provider agencies comply with state and federal regulations. Employment screenings, as outlined in the Personnel Policy, Rules, and Procedures Manual, will be conducted in compliance with applicable federal and Florida Laws. Working Conditions The work environment may include exposure to high noise levels while performing certain essential job functions. The position requires the ability to lift and move objects weighing up to 50 pounds independently and up to 75 pounds with assistance, with or without reasonable accommodation. The role involves sitting for extended periods while working on a computer. Essential Functions These are intended only as illustrations of the various types of work performed. The omissions of specific duties does not exclude them from the position. Assists in conducting programmatic reviews of case files in order to ensure compliance with state and federal regulations and guidelines for which the County Commission acts as grantee. Assists in interpreting federal and state regulations and guidelines on programmatic grant recipients for whom the County Commission acts as grantee. Processes payments to customers based on receipts in the monthly home care to the elderly program. Acts in a contract manager roll when assessing compliance of service providers that have entered into agreements with the County Commission to provide Aging Services to the people of Manatee County. Pre-Screens referrals by telephone to determine initial eligibility. May conduct full assessment in client's home to determine final eligibility. Arranges and coordinates appropriate services for clients. Maintains appropriate client case files. Maintains contact with client in the home at a minimum of once every six (6) months, with more frequent when needed. Prepares monthly reports as needed. Maintains a case load which consists of review, assessments, referrals and follow-up. Attends and participates in all staff meetings, in-services, training and inter-agency meetings. Acts to assure that service is being provided to clients in an appropriate and adequate manner. Will assist in training new Case Manager I and/or case Manager II employees. Performs other related duties as assigned, including participation in emergency or disaster response activities, as required and consistent with the employee's qualifications, essential job functions, and appliable federal and Florida law. Bachelor's degree in social work or related to area of assignment, i.e., social welfare, psychology, sociology, counseling, gerontology, etc. Valid driver's license with valid Florida driver's license within 30 days of hire. Equivalent combinations of education, training, and experience may be considered in meeting the minimum qualifications for this position.
Knowledge, Skills, and Abilities:
Knowledge of social work principles, practices and procedures. Knowledge of the social service delivery system in the County and State. Ability to work with assigned special needs clients. Ability to establish and maintain effective working relationships with County staff, related agency staff, clients and the general public. Ability to communicate in a clear and concise manner. Skill in conducting interviews to elicit critical program data. Strong working knowledge of computer Windows systems and associated software. Ability to understand and follow oral and written instructions and communicate same in reports. Strong ability to apply known skills and knowledge to meet challenges and adapt to unusual situations.
