Job Description

Job Overview We are seeking a compassionate, detail-oriented Medical Case Manager to join our healthcare team. This role will focus on providing comprehensive case management services to patients living with HIV , with a strong emphasis on care coordination, treatment adherence, and housing support services . The ideal candidate has experience working with HIV clients, is knowledgeable in housing programs (HOPWA, subsidized housing, etc.) , and is committed to improving patient outcomes through coordinated, patient-centered care. Key Responsibilities Conduct comprehensive patient assessments (medical, psychosocial, housing, and support needs) Develop and implement individualized care plans to support patient health and stability Coordinate medical, behavioral health, and social services for patients, ensuring continuity of care Provide linkage to HIV care , treatment adherence counseling, and ongoing patient follow-up Assist patients with housing resources , applications, and maintaining housing stability Connect patients with community resources including transportation, food assistance, mental health, and substance use services Advocate for patients and assist with benefits enrollment (Medicaid, Medicare, Ryan White programs, etc.) Maintain accurate and timely documentation, case notes, and reports in compliance with HIPAA and program requirements Collaborate with providers, nurses, and community partners to ensure coordinated care delivery Maintain regular contact with patients to monitor progress and adjust care plans as needed Qualifications Bachelor's degree in Social Work, Public Health, Psychology, or related field (required) Minimum 1-3 years of case management experience , preferably in HIV/AIDS or healthcare setting Experience working with HIV-positive populations (required) Strong knowledge of housing programs and community resources Familiarity with Ryan White Program, HOPWA, and benefit programs (preferred) Knowledge of HIPAA, care coordination, and medical case management standards Excellent communication, organizational, and documentation skills Ability to manage caseloads and work independently in a fast-paced environment Preferred Skills Bilingual (English/Spanish or Haitian Creole) Experience with EHR systems / CAREWare or similar databases Strong background in housing navigation and patient advocacy Cultural competency and ability to work with diverse populations
Pay:
$25.00 - $30.00 per hour
