This position provides vital support to individuals and families seeking assistance, helping them access social services, resources, and programs to improve their quality of life.
Responsibilities include:
  • Interviewing clients to assess their needs and eligibility for available programs and services
  • Assisting clients in completing required forms and gathering documentation
  • Coordinating with case managers, social workers, and other staff to deliver comprehensive solutions
  • Referring clients to community resources such as employment programs, housing assistance, healthcare, and educational opportunities
  • Maintaining accurate records of client interactions and service delivery in accordance with agency policies
  • Monitoring client progress and following up to ensure ongoing support and successful outcomes
  • Participating in outreach activities and community events to raise awareness of available services
  • Supporting clients with advocacy, information, and guidance as needed
  • Upholding confidentiality and ethical standards in all client interactions Ideal candidates demonstrate strong communication, empathy, organizational skills, and the ability to work collaboratively in a fast-paced, diverse environment.
Bilingual (English/Spanish or English/Creole) candidates are encouraged to apply. Experience in social services or community programs is preferred.
