Job Description
About Mia Casa Mia Casa is a senior-focused shelter program dedicated to helping residents transition from homelessness into stable, permanent housing . Our program operates within the Miami-Dade County Homeless Trust Continuum of Care (CoC) system and focuses on housing stabilization, service coordination, and long-term housing outcomes . This is a housing-focused program , not a short-term triage shelter. Our goal is to help residents achieve lasting stability through housing readiness, service connection, and coordinated support. We are seeking an experienced Lead Case Manager (Working Lead) who can manage an active caseload while also supporting the overall coordination and quality of case management services. Position Overview The Lead Case Manager will manage a high-volume caseload of up to 30 residents while also providing guidance and oversight to case management staff. This role requires strong experience in housing-focused case management, HMIS documentation, and Continuum of Care compliance. Key Responsibilities Case Management Manage an active caseload of up to 30 residents Conduct assessments and develop housing stabilization and service plans Prepare residents for housing placement by ensuring documentation, benefits, and services are in place Connect residents to healthcare, behavioral health services, and community resources Coordinate transitions to permanent housing, supportive housing, or other appropriate placements Program Support & Leadership Provide day-to-day guidance and support to case management staff Review service plans and documentation for quality and compliance Support team coordination to ensure housing goals and program outcomes are met Compliance & Data Management Ensure accurate and timely HMIS data entry Maintain organized and audit-ready client files Support compliance with Miami-Dade County Homeless Trust and Continuum of Care standards Qualifications Required Bachelor's degree in Social Work, Human Services, or related field 3-5 years of experience in homeless services, supportive housing, or case management Direct experience working with HMIS Strong understanding of housing-focused case management Ability to manage a high-volume caseload independently Preferred Experience working within the Miami-Dade County Homeless Trust system Experience serving senior populations Familiarity with SSI, Medicaid, and waiver programs Previous experience as a
