Full Job Description Summary:
Provide direct services to residents; develop and implement individual case plans; assist residents in achieving goals; facilitate groups and activities; teach and/or assist residents in developing daily living skills, communication and self-advocacy skills; support the continuation of the resident council and other community activities; make referrals to community-based services; assist in accessing and maintaining entitlements/benefits or employment; assist in obtaining permanent housing; and advocate for needed services. Essential Duties and Responsibilities include the following: other duties may be assigned. Develop an individualized case plan for each resident within the first ten days of program entry. Address overcoming barriers to maintaining transitional housing such as enhancing daily living skills, transportation, improving mental and physical health, substance abuse recovery, obtaining GED and education, income augmentation, job skills and personal budgeting. Assist residents in applying for food stamps, reduced fair bus pass, new ID with current address, healthcare benefits and SSI/disability benefits, housing within 30 days of arrival. Make referrals to outside agencies to assist residents in meeting case plan goals. Complete timely documentation to ensure ongoing compliance with internal and external requirements. Enter and maintain resident information in HMIS computer network. Prepare and maintain resident files for compliance audits. Meet with residents monthly and as indicated to review the individual's case plan and initiate new goals. Assist in locating permanent affordable housing. Meet as necessary with residents to address behavior problems and/or relapse issues. Read staff's notes regularly. Maintain ongoing awareness of each resident's needs. Collaborate with team members to aid in development of resident's case plan. Assist residents to identify and resolve problems and make effective use of resources. Encourage their independence and increase self-esteem. Conduct Intake for potential program participants. Communicate verbally and in writing with healthcare and social service agencies and attorneys as needed regarding resident issues. Schedule and coordinate agency appointments. Assess needs of program and discuss. Initiate interventions and evaluate effectiveness. Conduct relevant groups and classes with residents and staff. Attending required training and meetings.
