The SSVF Case Manager provides housing stability case management and supportive services to low-income Veteran families who are homeless or at risk of homelessness. The primary goal is to rapidly re-house homeless Veterans and prevent homelessness among those at imminent risk through client-centered, strengths-based case management. This position requires a deep understanding of social work principles, crisis management, and behavioral health to effectively assist clients in navigating their challenges. The Case Manager will work collaboratively with various stakeholders to ensure clients receive comprehensive support tailored to their unique needs. Duties Develop individualized housing stability plans in collaboration with clients. Coordinate and facilitate rapid re-housing and homelessness prevention services, including housing location, landlord negotiation, and rental assistance. Maintain a caseload of program participants and conduct regular follow-ups (in-person, by phone, or virtual). Assist clients in accessing public benefits and community resources, including VA services, employment, healthcare, and legal assistance. Document all case notes, services, referrals, and outcomes in the HMIS system and internal databases. Collaborate with landlords, housing authorities, shelters, VA staff, and other community partners. Attend team meetings, case consultations, and required trainings. Ensure compliance with SSVF Program Guidelines, funder requirements, and agency policies.
Qualifications:
Required:
Bachelor's degree in Social Work, Psychology, Human Services, or a related field. At least 2 years of case management or social services experience. Strong understanding of issues related to homelessness, mental health, and trauma-informed care. Excellent communication, organization, and problem-solving skills. Proficiency in Microsoft Office and experience with data entry. Valid driver's license, reliable transportation, and willingness to travel locally.Preferred:
Experience working with Veterans or in a housing-first model. Knowledge of HMIS and VA systems (e.g., SQUARES, HSR).Working Conditions:
