Full Job Description Agency Overview:
DISC Village, a non-profit community-based agency, has been in operation for over 50 years and is one of the largest and most comprehensive prevention, intervention and treatment agencies in Florida. Through the years, DISC Village has become recognized as a leader in the state for initiating, managing and coordinating substance abuse prevention/intervention, family welfare and delinquency programs in Leon County and North Florida. Selected Family Interventions (SFI) is seeking a Counselor, dedicated individuals to provide in-home intervention services to families throughout the community in Gadsden and Liberty Counties. Services include assessment, family counseling, individual counseling and case management for those involved in the child welfare system and have experienced or have been affected by trauma. The SFI Program works collaboratively with Child Protective Investigators (CPI) and Dependency Case Managers (DCM) to engage caregivers and increase their ability to care for the family. Essential Responsibilities Provide individual and family intervention services for families while following the SFI manual approach to services. Services will be provided in various settings but not limited to: in-home, office, and other community locations. Services may be provided on evenings as needed. Administer comprehensive intake paperwork and biopsychosocial assessments. Document assessment findings and referral recommendations. This includes utilizing the FSFN network and documenting all delivered services within the specified SFI timeframes. Collaboratively develop individualized intervention plans and review the plan monthly with the person served. Monitor the quality of care/reassess need for service. This involves consistently monitoring the progress of the person served in order to make a determination whether additional services are needed. Coordinate any necessary individualized services to ensure appropriate, effective, and timely interventions are initiated. Conduct weekly sessions with each client based on their identified goals.
Knowledge, Skills and Abilities:
Strong verbal and written communication skills. Performs work independently with minimal supervision. Capable of multi-tasking, highly organized, with excellent time management skills. Strict adherence to confidentiality with high ethical standards. Ability to create and maintain professional relationships within all levels of the organization Understand the program principles/philosophy of Trauma Informed Care (TIC) and Recover Oriented System of Care (ROSC).
Qualifications:
Master's degree in a social, behavioral, or related field. Preferred experience in mental health, substance misuse, child welfare or related field.
