reviews $21 an hour - Full-time COACHES Targeted Case Manager (TCM) St. Augustine Youth Services - St. Augustine, FL Part-Time | 32 Hours/Week | Onsite Competitive Pay + Meaningful Impact Make a Difference in the Lives of Youth and Families St. Augustine Youth Services is seeking a compassionate, organized, and driven Targeted Case Manager (TCM) for our COACHES program. This role is ideal for someone passionate about helping children and families navigate behavioral health challenges while connecting them to meaningful community resources and support. As a Targeted Case Manager, you will serve as a key advocate and coordinator for children with serious emotional disturbances, helping ensure they receive the services and support needed to thrive. What You'll Do Provide the five core functions of case management: Assessment Planning Linking & Advocacy Monitoring Discharge Planning Conduct initial assessments and develop service plans in accordance with Florida Medicaid guidelines Coordinate services with families, schools, treatment providers, and community agencies Attend required meetings including MDT, IPT, school meetings, and provider staffing Monitor progress and evaluate service effectiveness Connect clients and families with needed community resources Provide crisis intervention and support as needed Maintain accurate and timely documentation in client records and electronic systems Complete service plans, progress reports, and other required documentation . Qualifications Required Education & Experience (Must meet one of the following): ✔ Bachelor's degree in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or related human services field AND at least 1 year of experience working with children with serious emotional disturbances OR ✔ Bachelor's degree in any field AND 3 years of full-time experience working with children with serious emotional disturbances Preferred Skills Knowledge of community resources and service systems in the St. Augustine area Understanding of Florida Medicaid and behavioral health documentation requirements Strong communication and relationship-building skills Ability to work collaboratively with multidisciplinary teams Strong organization and time management Ability to advocate effectively for youth and families What Success Looks Like Successful candidates will: Maintain wraparound meetings every 60-90 days Meet documentation and compliance standards at 80% or higher Successfully connect families to services and supports in at least 90% of discharge plans Why Join SAYS?
