The Case Manager is responsible for supporting the mission and core values of Caring & Sharing of South Walton by performing comprehensive assessments to clients who are experiencing a crisis. In this position the case manager will utilize the information from the comprehensive assessment to provide immediate support and guidance to help stabilize their situation by helping to identify key barriers, connecting them with relevant community resources, and providing ongoing support through their crisis.
Client Programming:
Conduct comprehensive needs assessments for clients during their first visit to identify immediate requirements and develop appropriate intervention plans. Connect clients with relevant community resources, including employment assistance, housing support, healthcare services, and additional social services to address barriers identified during the needs assessment. Guide clients through application processes for government assistance programs such as SNAP, TANF, and Medicaid, ensuring proper documentation and submission. Support food distribution operations during scheduled assistance periods as assigned, ensuring efficient service delivery and appropriate resource allocation. Manage and respond to client inquiries received via phone, email, and in-person contacts, providing timely and accurate information about available services. Coordinate effectively with staff members, volunteers, and external agencies to ensure integrated service delivery and avoid duplication of efforts. Establish and nurture relationships built on trust and respect with clients, volunteers, vendors, landlords, and organizational partners to facilitate effective assistance. Maintain current knowledge of internal policies, services, and programs, as well as external resources available to support clients in crisis, ensuring alignment with organizational mission. Monitor program expenditures and resource utilization to maintain adherence to established assistance budgets and financial guidelines. Implement systems for tracking and reporting program impact metrics and client progress to demonstrate effectiveness and identify improvement opportunities. Collaborate with community partners to identify service gaps and unmet needs, recommending strategic program improvements to enhance assistance effectiveness. Provide crisis intervention services and participate in case conferencing with other service providers to develop comprehensive support plans for complex client situations.
Community Outreach:
Cultivate strategic relationships with local and regional organizations to enhance resource accessibility for low-income individuals and families as well as those experiencing homelessness. Serve as an official representative for Caring & Sharing of South Walton at community outreach events and coalition meetings. Collaborate effectively with other case managers and external service providers to ensure seamless delivery of client services. Required Qualifications Must successfully pass a Level 2 Background Check prior to employment. Strong communication and interpersonal skills with the ability to connect with clients from a variety of backgrounds. Ability to conduct needs assessments and develop appropriate support plans. Reliable transportation with the ability to travel locally for job-related duties. Preferred Qualifications The following are preferred but not required. We encourage applicants from all backgrounds to apply. Bachelor's or master's degree in social work, human services, or a related field. Experience working with low-income or homeless populations. SOAR certification. Experience using HMIS (Homeless Management Information System). Training in trauma-informed care, mental health first aid, or crisis intervention. Experience in volunteer or program management. Public speaking experience. Ability to speak Spanish. Knowledge, Skills, and Abilities Self-motivated with strong organizational skills. Attention to detail and ability to track client progress effectively. Ability to work collaboratively within a team environment. Strong problem-solving and critical-thinking skills. Commitment to the mission and values of Caring & Sharing of South Walton. Ability to manage time effectively and adapt to changing client needs. Strong communication and advocacy skills. Working Environment Caring & Sharing of South Walton operates within a food pantry, warehouse, and retail setting. Work may be performed indoors and outdoors in varying weather conditions. Physical Requirements This position requires mobility, standing, walking, and occasional lifting of objects up to 25 pounds.
