Job Description
Provides case management and care coordination for Early Intervention Program (EIP) and Part C eligible children. Reviews charts to determine program eligibility. Completes CMS related documents. Performs as a member of a multi-disciplinary team to identify patient medical and developmental needs. Develops Family Support Plans and assists families in meeting their goals via providing short-term parenting intervention or community referrals. Makes home visits, when necessary. Documents all related work. This position will require a level II background check. This position is time limited.
Minium Qualifications:
This position requires a high school diploma or equivalent, with three years' experience in office, administrative, or customer services positions. Appropriate college coursework or vocational/technical training may substitute at an equivalent rate for the required experience.
Preferred Qualifications:
At least two years professional experience in respective discipline. Knowledge of community resources. Bi-lingual (English/Spanish) preferred. A Bachelor's or Master's degree in public health, social work, special education or related clinical discipline is preferred. Substitution of Work Experience for Postsecondary Education Requirements A public employer may include a postsecondary degree as a baseline requirement only as an alternative to the number of years of direct experience required, not to exceed:
(a) Two years of direct experience for an associate degree;
(b) Four years of direct experience for a bachelor's degree;
(c) Six years of direct experience for a master's degree;
(d) Seven years of direct experience for a professional degree; or
(e) Nine years of direct experience for a doctoral degree Related work experience may not substitute for any required licensure, certification, or registration required for the position of employment as established by the public employer and indicated in the advertised description of the position of employment.
Minimum Qualifications that require a high school diploma are exempt from SB 1310. Intake & Eligibility Assessment (30%) Conducts intakes of children and families entering the Early Intervention Program including assessment of program eligibility, psychosocial history, and demographic information. Assist family in scheduling an initial clinic appointment for medical and/or developmental assessment. Intakes may be conducted in a variety of settings: home, clinic, community agencies/organizations. Case Management & Family Support Planning (30%) Reviews and documents medical history form for children, birth to three, to determine Part C Program eligibility. Develops Family Support Plans in collaboration with members of a multi-disciplinary team in the Early Intervention Program Implements intervention plans to teach families positioning techniques, understanding infants stress cues, feeding techniques, etc. per Family Support Plans until appropriate community services are arranged. Provides targeted case management and care coordination services for a caseload of Part C eligible children and families. Completes required CMS and EIP forms with patients and bills Medicaid as required. Care Coordination & Family Support (20%) Coordinates and assists parents in making clinic and agency appointments. Responsible for assuring patients complete recommended medical/developmental evaluation for a caseload of Part C eligible children/families. Arrange transportation when necessary. Reports scheduled appointments to other team members as necessary and documents in chart. Telephone liaison with health care and insurance providers under supervision of a physician. Documentation & Records Management (10%) Documents contacts in patient EIP records and the Family Support Plan. Community Outreach & Education (5%) Coordinates and implements periodic in-service teaching regarding Part C issues for hospitals/community providers served by the EIP Program. Additional Duties (5%) Completes other duties as required by EIP Program.
