Job Description

Job Summary The Carter Treatment Center is seeking a full-time Case Manager in Alpharetta, GA. The Case Manager plays a crucial role in supporting individuals by providing comprehensive case management services. This position involves assessing client needs, developing personalized service plans, and coordinating resources to facilitate positive outcomes. The Case Manager will work collaboratively with clients, community organizations, and service providers to ensure effective support and advocacy. Duties Conduct thorough assessments of clients' needs and circumstances to develop tailored service plans. Meet with clients to engage in aftercare/discharge planning. Provide ongoing support through motivational interviewing techniques to encourage client engagement and empowerment. Implement crisis intervention strategies to address immediate client needs and stabilize situations effectively, ensuring the safety and well-being of clients. Supervise and mentor clients in various programs skill areas, including childcare initiatives and workforce development efforts. Collaborate with social work and counseling professionals to address behavioral health issues and connect clients with necessary resources. Maintain accurate documentation of client interactions, progress notes, and service plans in compliance with organizational policies. Advocate for clients' rights and access to services, including assistance with housing applications when applicable. Engage in group facilitation with clients. Communicate and collaborate with a multi-disciplinary treatment team. Other duties, as required. Qualifications Experience working with individuals with substance use and mental health disorders, preferred. Experience working in partial hospitalization (PHP) and intensive outpatient (IOP) programs, preferred. Must have care coordination skills to assist individuals with accessing/linkage to additional resources and services within the organization and/or community. Excellent communication skills for building rapport with clients, families, and community partners. Strong organizational skills to manage multiple cases efficiently while maintaining attention to detail. Competent in the use of technology, including Microsoft Office, Outlook, and EHR systems. Ability to pass a background check.
Salary :
$50,000-60,000 per year, dependent on education and experience; benefits available for full-time employees.

Full-time, Monday through Friday 8:30-4:30 (schedule to be discussed further)
