Summary:
Preventive Measures is seeking a bilingual case manager (Spanish/English). Our Case Managers provide essential support, psychosocial individual and coordination of comprehensive care services for a diverse caseload of clients (children, adolescents and adults) dealing with mental health and substance abuse issues. This role will aid clients in receiving integrated, client-focused care and support across all aspects of their treatment and recovery process. As a Case Manager, you will be instrumental in linking clients with the appropriate resources, enhancing skillsets, providing psychoeducation, monitoring their progress, and advocating for their mental health and physical health needs.
Primary Job Responsibilities:
Assist in assessing client needs and supporting their overall resources and skills needed are reflected on the Individual Recovery Plan (IRP). Provide direct support to clients in community and office settings, assisting with daily living skills, medication adherence, and access to community resources. Help coordinate services with providers and aid in access to essential treatments and resources. Maintain communication with clients, families, and service providers to monitor progress and address regress. Maintain a minimum of two monthly contacts face-to-face and virtual. Maintain confidential records of client interactions and progress notes. Submit all documentation within 48 hours of completing the service. Attend supervision and participate in multidisciplinary team meetings to provide input regarding client cases and updates (i.e. observations, challenges, progress, etc.). Provide logistical support, including scheduling appointments and arranging transportation. Assist with client outreach and follow-up activities to encourage continuity of care. Help educate clients and families about community resources and how to access them. Perform administrative duties related to case management and client care. May require weekend, evenings or adjusted hours due to program/client needs.
Qualifications:
Fluent in Spanish and English Bachelor's degree in Social Work, Psychology, or related field; Master's degree preferred. Proven experience in case management, preferably in mental health or substance abuse. Strong organizational and multi-tasking skills. Excellent communication and interpersonal skills, with an ability to interact effectively with diverse populations. Knowledge of community health resources and services. Commitment to maintaining confidentiality and adhering to ethical standards. Valid driver's license and reliable transportation.
