Job Description

Pineland BHDD is currently hiring Case Managers for our Wayne Counseling Center in Jesup, GA. This role is ideal for compassionate professionals who want to support individuals through coordinated care, advocacy, housing support, and community linkage within a structured residential treatment setting. What You'll Do As a Case Manager, you'll provide comprehensive, person-centered social services while serving as a key member of the interdisciplinary treatment team. Core Responsibilities Provide ongoing case management Maintain regular face-to-face contact with individuals on your caseload Serve as a liaison between individuals, families, providers, and community agencies Coordinate referrals and link individuals to housing, healthcare, and recovery supports What We're Looking For Preferred Education Bachelor's degree in Behavioral Health, Counseling, Psychology, Social Work, Rehabilitation, Criminal Justice, or a related field Required Valid Georgia Driver's License At least 21 years of age Ability to travel locally and conduct home or community visits Skills That Will Help You Succeed Strong case management and documentation skills Ability to de-escalate crisis situations with empathy and professionalism Experience working with individuals with substance use disorders Knowledge of evidence-based practices Strong organizational, communication, and time-management skills Commitment to ethical practice, confidentiality, and HIPAA compliance Why Work at Pineland BHDD?
