Case Manager

We are seeking a motivated and organized Case Manager to join our growing team. Position Summary The Case Manager is responsible for coordinating client care services, developing care plans, and ensuring clients receive appropriate support tailored to their needs. This role serves as a key liaison between clients, families, caregivers, and healthcare professionals. Key Responsibilities Conduct initial client assessments and ongoing evaluations Develop and update individualized care plans Coordinate and schedule home care services Communicate regularly with clients, families, and caregivers Monitor client progress and ensure satisfaction with services Maintain accurate and timely documentation Ensure compliance with company policies and state regulations Address client concerns and resolve issues promptly Qualifications Bachelor's degree in Social Work, Healthcare Administration, Nursing, or related field (preferred) Previous experience in case management, home care, or healthcare setting Strong organizational and communication skills Ability to multitask and manage a caseload efficiently Knowledge of home care regulations and best practices Valid driver's license and reliable transportation Skills & Competencies Compassionate and client-focused approach Strong problem-solving skills Attention to detail Ability to work independently and as part of a team Proficiency with electronic health records (EHR) systems
