Job Description

St Illa CSU Closing date: Open until filled Hourly rate: $18.33
Job Description:
Responsible for working with the treatment team and client to establish and implement effective discharge plans. Participates in treatment team planning and documents in charts. Discharge planning includes scheduling follow up outpatient appointments, medical appointments if necessary, assisting with finding placement if needed and other linkage as determined by the client's needs and treatment team. Responsible for follow up calls to check if client followed up with discharge referrals/appointments. Assist with preparing and faxing paperwork to the pharmacy for discharge medications. Job is primarily for the Crisis Stabilization Program but may be asked to assist with the Temporary Observation Unit.
Job Responsibilities:
Will work Monday-Friday 8 am - 5 pm
Competencies:
Knowledge of general office procedures. Knowledge of agency confidentiality policies. Ability to communicate orally and in writing. Ability to determine customer's issues and handle accordingly. Ability to work with little or no direction. Ability to prioritize and meet deadlines.
Required Minimum Qualifications:
High school diploma or equivalent and three years' experience in a social service-related position OR associate's degree in a social service-related field and one year experience in a social service-related position. Must be at least 21 years old to apply. Must possess a valid driver's license and have a clean driving record with less than 6 points.
