We are seeking a compassionate, motivated Community-Based Case Manager to join our growing team. This is a field-based position focused on recovery-oriented, trauma-informed care delivered directly in the community. CER is a mission-driven nonprofit behavioral health organization providing comprehensive services to individuals experiencing serious and persistent mental illness (SPMI), co-occurring substance use disorders, and housing instability. As a Case Manager, you will build clinically informed relationships with adults diagnosed with SPMI and support them in achieving stability, independence, and improved quality of life through individualized recovery planning and coordinated services. Caseloads are intentionally kept low (approximately 10-12 clients) to ensure meaningful engagement and high-quality care. Key Responsibilities Develop and maintain therapeutic, recovery-oriented relationships Create and implement individualized service and recovery plans Coordinate psychiatric, medical, housing, legal, vocational, and substance use services Provide support in homes and community-based settings Transport clients when clinically appropriate and safe Collaborate with treatment providers and community partners Maintain accurate and timely documentation in compliance with agency and funding requirements
WE SUPPORT OUR CASE MANAGERS
We are a close-knit agency, and you will have ample support from your supervisors. Your case management team includes mentors, leaders and supervisors with an open-door policy and licensed professionals who are always available to consult with you. Additionally, you will have regular team meetings, clinical supervision with your supervisor, and ongoing behavioral health training. We provide ample training on evidence-based practices and community resources. Our approach to treatment is highly team oriented, so you always have the support of your peers and supervisors. Requirements We welcome both early-career professionals and experienced clinicians.
Preferred:
Bachelor's degree in Psychology, Social Work, Health, Nursing, Counseling, or related field (with coursework in psychology)
Required :
6-12 months of supervised experience (employment, internship, volunteer, or combination) working with at least one of the following populations: SPMI, DD, Elderly, Children or other special needs population. A combination of relevant education and experience accepted Strong time management, communication and organizational skills Comfortable working independently and autonomously in community settings
Required:
Access to a reliable vehicle, valid driver's license, clean driving abstract, and valid vehicle safety check Benefits Up to $65,000+ annually in the first year, depending on personal productivity Hourly, fee-for-service structure with opportunity for overtime pay PPO health insurance plan fully paid by CER (medical, dental, vision, prescription coverage) 401(k) with opportunity for annual agency contribution Paid vacation and sick time Monthly cell phone stipend One-time stipend toward purchase of a tablet/device Fuel/Gas stipend All state and federal holidays observed
