CASE MANAGER About This Role The Beacon at Sherman Hill is seeking a compassionate, trauma-informed Case Manager to join our team. In this role, you will provide individualized support to women navigating crisis recovery, personal development, and long-term stabilization, walking alongside residents as they rebuild their lives. Standard business hours; occasional evenings or weekends for house meetings or resident events Work Setting:
This is a fully on site position What You'll Do Client Support and Case Management Manage a caseload of up to 36 women in residential programming, providing trauma-informed, individualized support Develop and monitor client-centered case plans that are measurable, goal-oriented, and adaptable Conduct client intakes, assess needs, and establish safety and stabilization plans Provide crisis intervention, advocacy, and supportive counseling with a compassionate, non-judgmental approach Facilitate client navigation through program phases, supporting progress toward personal milestones Assist residents in accessing housing, employment, education, healthcare, and community resources Hold regular one-on-one meetings to review progress, adjust case plans, and provide encouragement and accountability Documentation and Data Management Maintain accurate resident and service data in compliance with funder and organizational requirements Support outcome tracking, analysis, and reporting to improve services Participate in internal and external audits, ensuring documentation accuracy and compliance Collaboration and Advocacy Collaborate with Resident Assistants, support staff, and leadership to maintain a safe, respectful environment Advocate for clients within The Beacon and with external service providers Participate in multidisciplinary team meetings, case reviews, and staff training sessions Coordinate The Beacon Alumni Program, maintaining relationships with graduates and tracking ongoing outcomes Attend every other house meeting in rotation with the other Case Manager Community Engagement and Program Facilitation Facilitate group meetings, workshops, and life skills training aligned with the eight dimensions of wellness Coordinate with external partners to bring educational programming on site Represent The Beacon at community events, resource fairs, and partner meetings What We're Looking For We are looking for someone who is ready to show up fully, not just for the work, but for the women. We are open to candidates who bring a mix of formal education, lived experience, and hands-on work. Experience Case management or similar experience in social services, crisis intervention, housing, or a related field Lived experience with trauma, substance use recovery, or housing instability, whether personal or through a loved one Experience working with underserved populations and a solid understanding of trauma-informed care Skills and Knowledge Strong organizational and time management skills to balance administrative and client-facing work Calm, solution-focused approach in high-stress situations Excellent verbal and written communication and advocacy skills Understanding of trauma-informed, equity-centered, and evidence-based practices Proficient in Microsoft Office Suite (Word, Excel, Outlook) or ability to demonstrate proficiency within 90 days Familiarity with HMIS or OneStep is a plus but not required; training will be provided Valid driver's license and reliable transportation Why The Beacon?
