Job Description

Overview Join our dynamic Caldwell team as a Case Manager, where you will play a vital role in our mission as you provide client advocacy, community resource education and coordination, and drug-test monitoring. This energetic position offers the opportunity to utilize your critical thinking and compassionate communication skills to improve client outcomes for those in our various behavioral health programs (mental health and substance use treatment). As a Case Manager, you will collaborate with multidisciplinary teams, manage complex cases, and facilitate efficient utilization of resources to deliver high-quality care. Responsibilities Manage and coordinate a caseload providing client advocacy, community resource connections, and wrap around services. Coordinate resources and referrals with community partners. Assist clients in meeting basic needs such as housing, employment, accessing insurance including Medicaid and other state assistance. Monitor client attendance and treatment progress and participate as a member of the treatment team. Complete case management assessments and individualized service plans. Complete monthly progress reports for external partners such as probation officers. Network with community resource partners and maintain a current database of resources. Experience Strong understanding of accessing and navigating community resources. Embrace the ability to change and recognize possibility. Experience in behavioral health and/or human service. Experience in case management with the ability to coordinate care, problem solve, and build relationships. Skilled in utilizing electronic health record systems efficiently. Demonstrated ability to manage complex cases involving social work considerations, discharge planning, and utilization management. Excellent communication skills with a solid understanding of behavioral healthcare terminology and healthcare regulations such as HIPAA. Qualifications Bachelor's degree from an accredited college or university in a human services field. Valid driver's license and a driving record that is acceptable to the company. Ability to pass state and federal background checks.
Pay:
$21.00 - $25.00 per hour
