JOB DESCRIPTION
SUMMARY
OF POSITION
Connects with clients and partners in the community to provide education and linkage to the TREAT program. The Outreach Clinician will be responsible for developing relationships with schools in the catchment area, organizing training opportunities with partners, and assisting with the coordination of the Advisory Council. Completes assessments, conducts crisis evaluations, and maintains a small caseload as indicated. Provides case management services.
ESSENTIAL DUTIES & RESPONSIBILITIES
Works as a liaison in communities to promote positive collaboration between Centerstone and community partners. Develop and maintain relationships with local service and community resource agencies that can benefit participants. Works closely with other team members to identify the needs of the community and clients who are participating in the program. Provides transportation assistance as needed to help participants achieve and maintain case management goals. Disseminates educational information through presentations to local stakeholders / community groups, youth, family/caregivers, law enforcement, local schools, agencies, court officials, and other community groups via telehealth and in-person across the southern Illinois region. Provides assertive outreach, advocacy, support and follow-up to assigned participants and documents all outreach attempts. Assists participant in building a natural support team. Integrates natural community supports/resources into the planning and service delivery process. Provide interventions that are recovery focused taking into account trauma informed care practices. Demonstrates the ability to help families and clients identify, select and practice strategies to maintain/obtain resources. Conducts assessments for clients entering services, including explaining services, gathering information needed on intake materials, ensuring service recipient understands all forms, assuring signature on appropriate contracts and consents, and submitting materials for processing, as assigned. Conducts assessment and write assessment summaries, as assigned. Assists clients in developing an individualized treatment plan, must be person centered, family centered, and service centered. Assists clients with making linkage with internal and external resources as outlined in treatment plan and in coordination with Integrated Healthcare Services. Maintains appropriate contact with referral sources including providing information on service recipient progress, behavior, and needs (with appropriate releases), and maintaining ongoing consultation when appropriate.
KNOWLEDGE, SKILLS & ABILITIES
Effectively communicate via written, verbal, in person and virtual methods. Knowledge of community geographical awareness and of community and agency resources. Skill in customer service techniques. Skill in developing problem solving strategies. Skill in consultation and networking.
QUALIFICATIONS
Education Level Bachelor's degree in Social Work or Psychology. Master's Degree preferred Years of Experience Two years of experience preferred Required Certification/Licensure Licensure (LPC or LSW) or CADC certification preferred or must be willing to obtain license or certification within 2 years of hire.
