Area of Focus:
Behavioral Health Program(s): ReLeaf- Is a transitional housing program providing trauma-informed, no-cost housing and wraparound support services for survivors of domestic violence, sexual assault, human trafficking, and community violence across Southern Illinois.
Immediate Supervisor:
Program Manager (once hired) In the intrum, Program Director•Daniel Hyson Hours of Work:
Full-time 40 hours/ week. Crisis sitter on-call rotation.FLSA Classification:
Non-exempt Case Manager Minimum Qualifications :
Bachelor's Degree in Human Services or related field, qualifies as a Mental Health Professional (MHP). Previous case management experience is preferred.Case Manager Special Requirements:
Must be 21 years of age or older to drive agency vehicle; maintains a valid driver's license and vehicle liability insurance; operates agency vehicles and personal vehicles while driving for agency business in a safe and lawful manner at all times; maintain First Aid & CPR certification; must complete required registry clearances and criminal fingerprint background checks; schedules evening or weekend hours, as necessary; participates in crisis sitter on-call rotation.Case Manager Skills & Competencies:
Knowledge of local health and human services resources. Excellent active listening skills. Skilled in the delivery of case management/care coordination services. Basic crisis intervention and conflict resolution techniques. Excellent written, oral, visual, and non-verbal communication skills. Ability to work independently and collaboratively with team members. Excellent organizational skills. Excellent time management skills and the ability to meet deadlines as assigned. Case Manager j ob duties include, but not limited to: Provide individualized, trauma-informed case management, safety planning, and advocacy for victims of crime and their families. Complete intakes, assessments, and service plans that support housing stability, safety, and self-sufficiency. Maintain consistent client contact (minimum of 9 contacts per quarter), including weekly engagement to monitor safety, housing stability, and progress toward goals. Assist clients with accessing and coordinating services, including behavioral health, medical care, legal assistance, housing resources, and community supports. Provide or coordinate supportive services including counseling referrals, housing advocacy, employment assistance, education support, financial literacy, transportation, and life skills development. Support clients in securing and maintaining safe, stable housing and transitioning to permanent housing. Track client progress, service delivery, and outcomes in the agency's EHR system in a timely and accurate manner. Maintain complete, accurate, and audit-ready documentation in compliance with VOCA, ICJIA, and agency requirements. Monitor progress toward program goals and ensure service delivery aligns with required performance measures and grant objectives. Collaborate with program staff and community partners to ensure coordinated, wraparound care. Participate in team meetings, supervision, and ongoing training to support effective service delivery. Assist with outreach and public awareness activities as needed to promote victim services. Uphold trauma-informed, client-centered practices that ensure safety, confidentiality, and dignity. Participate in on-call rotation to support crisis sitter needs at Deaconess Union County Hospital. Perform other duties as assigned to support program operations and client success.
