This role is ideal for someone who thrives in a fast-paced, field-based environment and enjoys building strong relationships with hospital partners. As an ECM, you will play a key role in driving admissions by coordinating patient transitions from hospitals to our facilities. Responsibilities for External Case Manager (ECM): Build and maintain relationships with hospital case managers, discharge planners, and social workers Serve as the main point of contact between hospitals and Complete Care facilities Coordinate patient referrals and ensure smooth transitions to post-acute care Conduct daily in-person visits to hospitals in the Chicago area Partner with admissions, clinical, and leadership teams to support census goals Track referral activity and identify opportunities for growth Qualifications for External Case Manager (ECM): Experience in healthcare admissions, marketing, case management, or hospital liaison role Skilled in relationship-building and communication Knowledge of hospital discharge planning and post-acute care preferred Self-motivated and able to work independently in the field Willingness to travel locally within the Chicago region Clinical background (RN/LPN) is a plus, but not required Why Join Complete Care?
