Job Description

Join Our Team as a Qualified Intellectual Disability Professional (Case Manager) in Chicago, IL! Are you passionate about making a positive impact in the lives of individuals with intellectual disabilities? Do you have the skills and drive to advocate for and support those in need? Active Visions is seeking a dedicated Qualified Intellectual Disability Professional to provide support to adults receiving CILA services through Active Visions.
Job Responsibilities:
Oversee a caseload of 12-15 individuals to ensure they are safe, healthy and an active participant in their community Provide case management services for individuals with intellectual disabilities Develop individualized Implementation Strategy Plan based on assessments and evaluations Coordinate and monitor services and supports for each individual Advocate for clients' rights and preferences Collaborate with a multidisciplinary team to ensure comprehensive care Provide crisis intervention and problem-solving support Maintain accurate and up-to-date documentation Supervise Direct Support Professionals working in
CILA Qualifications:
Bachelor's degree in social work, psychology, human services, or related field At least one year providing direct services to individuals with intellectual disabilities Knowledge of relevant regulations and government benefits Strong communication and interpersonal skills Ability to work independently and as part of a team Passion for advocating for vulnerable populations If you're ready to take on a rewarding and challenging role as a Professional (Case Manager) in the field of intellectual disabilities, we want to hear from you!
