Job Summary Join Our Team as an Adult Protective Services Case Manager! Serving Stephenson and Jo Daviess Counties | Full-Time Position Are you passionate about making a real difference in the lives of vulnerable adults? The Senior Resource Center (SRC) is looking for a compassionate, driven, and detail-oriented Adult Protective Services Case Manager to join our dynamic team. We serve adults 60+ years of age and adults with disabilities ages 18-59 , helping them maintain their independence, safety, and well-being in the community. About the Role As a Case Manager, you'll play a vital role in our mission by: Conducting intake and investigations of abuse, neglect, and exploitation Drafting and implementing individualized service plans Monitoring case progress and determining case closure Collaborating with service providers, law enforcement, and families Maintaining accurate documentation and adhering to state guidelines What We're Looking For We're seeking candidates with the following: Strong problem-solving , organizational , and time management skills Excellent verbal and written communication Ability to multi-task and adapt to a fast-paced environment Proficiency in Microsoft Office and use of remote computer equipment A Bachelor's degree in Psychology, Social Work, or a related Human Services field (preferred) Relevant human services experience (strongly preferred) A commitment to serving vulnerable adults with respect, empathy, and professionalism Training & Support State certification training will be provided Ongoing professional development opportunities Supportive team environment focused on collaboration and impact How to Apply If you're ready to make a difference, we want to hear from you!
