Job Description

Crisis Intervention Case Manager, Human Trafficking, Monday-Friday, 8am-4pm The Crisis Intervention Case Manager for our Human Trafficking, victim's services department, will provide mobile, on-site, trauma informed, survivor-centered crisis intervention and support to victims of human trafficking. This position is dedicated to crisis intervention and deployment during Illinois State Police recovery operations to provide on-scene stabilization, emergency shelter coordination, and trauma-informed support and resources. This position also participates in the medical advocacy pool, providing support to clients at the hospital after an assault. The Crisis Intervention Case Manager will respond and support and to crisis calls, referrals and walk-ins involving suspected or confirmed human trafficking situations within multiple settings, such as hospitals, law enforcement scenes, crisis hotlines, shelters, etc. They will provide client Identification and Intake by providing client trauma informed screenings and assessments for human trafficking and utilize evidence-based identification tools, and assess immediate safety, housing, medical, and legal needs. Furthermore, they will develop individualized service plans focused on safety, stabilization, and long-term independence and maintain regular contact with clients to monitor progress toward service plan goals. This includes advocating for survivors' rights and access to services within community systems; werve as a liaison between clients and community service providers; coordinate with law enforcement, healthcare providers, legal services, and social service agencies to support client needs (transportation, housing/shelter, medical, basic needs, etc.); and accompany clients to court hearings, medical appointments, housing or other advocacy meetings when appropriate. Skillsets with documentation and reporting should be exemplary in order to collect, maintain, and document client interactions and services in agency data systems in an accurate and timely manner and in accordance with program and grant requirements. A bachelor's degree in a related field is preferred.
