These services, which adhere to the guidelines set by Youth Services Network, Inc. and state policies overseen by the Redeploy Supervisor, are provided both at home and within the community.
Job Summary:
We seek a Bachelor's level applicant to provide case management services to identified adolescents who are actively serving a term of Juvenile Probation. Services are provided in collaboration with the Winnebago County Juvenile Probation Department. Case management services may include school advocacy, family support, linkage to community resources, treatment planning, and more. The candidate is expected to be trauma-informed and prepared to provide services to families and youth involved in the juvenile justice system.
Requirements:
Minimum of a Bachelor's Degree in a human service-related field. Minimum of two (2) years experience working with children/adolescents. Valid driver's license, proof of automobile liability insurance, and access to an automobile to transport clients.
Skills:
Proficient in both verbal and written communication. Familiarity with behavior issues, the juvenile justice system, wraparound services, and community resources. Demonstrate problem-solving techniques. Ability to exercise discretion and ensure confidentiality. Capable of handling high-stress situations. Works well in a team environment.
Duties/Responsibilities:
Provide ongoing case management services, including family support, school advocacy, linkage to community agencies, treatment planning, transportation, and more. Document all contacts with or on behalf of clients and maintain updated case files. Complete required assessments and reports. Effectively collaborate with outside agencies including the Winnebago County Juvenile Probation Department. Participate in local and statewide Redeploy meetings as directed. Become knowledgeable about the Juvenile Court Act and other State mandates that deal with the juvenile justice system. Attend all scheduled staff meetings. Maintain confidentiality of all client information. Participate in trainings, conferences, and workshops to enhance skills necessary for the position. Perform other related duties as assigned.
