Join Stepping Stones of Rockford, Inc. as an Outpatient Case Manager and become a vital part of a compassionate team dedicated to supporting individuals on their behavioral health journey. What You'll Do As an Outpatient Case Manager, you'll provide advocacy, guidance, and care coordination to help clients access the resources they need to thrive. You'll work closely with individuals, families, healthcare professionals, and community agencies to create meaningful change and ensure comprehensive support. Key Responsibilities Empower and Support Clients Help individuals develop resilience and self-management skills using evidence-based approaches. Partner with clients to create achievable goals and individualized care plans. Provide education and guidance to clients and their families regarding services, benefits, and community resources. Assist clients during crises and connect them with appropriate supports to maintain treatment engagement. Coordinate Comprehensive Care Collaborate with therapists, nurses, physicians, social workers, and community agencies to ensure quality care. Advocate for clients and help remove barriers to accessing services and entitlements. Connect individuals with community resources that strengthen natural support systems. Serve as a liaison between clients, providers, and public agencies. Maintain Excellence Complete accurate and timely documentation, progress notes, care plans, and Medicaid billing requirements. Monitor available resources and continuously seek opportunities to expand support options for clients. Adhere to HIPAA, Mental Health Code requirements, Rule 50 standards, and agency policies and procedures.
Requirements:
What We're Looking For We are seeking someone who is: Compassionate, empathetic, and committed to helping others succeed. An excellent communicator with strong interpersonal and problem-solving skills. Able to work independently while collaborating effectively within a team. Experienced in case management, care coordination, behavioral health, or related human services fields. Qualifications Bachelor's degree in a behavioral health, social service, or related field or five years of relevant experience. Knowledge of behavioral health treatment and community resources preferred. Valid Illinois driver's license in good standing. Reliable transportation and proof of liability insurance. Physical Requirements Ability to sit, stand, bend, and lift up to 20 pounds. Ability to drive agency or personal vehicles as needed for client services.
