Position Purpose:
The Case Manager for the Gennesaret Free Clinics' Health Recovery Homes (HRH) is responsible for providing intensive case management services to residents experiencing homelessness while recovering from acute or chronic medical conditions. The Case Manager works collaboratively with the Director of the Health Recovery Homes, the onsite Registered Nurse, Resident Care Assistants (RCAs), and community partners to support resident stabilization and recovery.
Case Management:
Conduct resident intakes, assessments, and individualized service planning. Ensure Coordinated Entry and Charity Tracker forms are completed for each resident. Provide ongoing one-on-one case management and resident advocacy. Assist residents with obtaining identification, benefits, housing applications, and community resources Assist residents with coordinating transportation (bus routes, bus passes, ride share, etc…) Provide transportation for social service, housing, and job search appointments\ Assist and coach clients with job searches, job placements, interview preparations, resume creation/modifications, and promote job retention. Participate in required staff meetings and case conferences Create and maintain relationships with service providers, property managers, landlords, staff, and community members to stay up-to-date with current resources for clients. Complete timely, accurate case notes and maintain resident documentation Support discharge planning and transitions Provide one year of monthly case management to clients who have exited the program.
Professional Expectations:
Adhere to all GFC policies and procedures while keeping the needs of the residents and the organization as the priority. Demonstrate leadership to gain and maintain credibility, trust, and respect of all clients, staff, and supervisors. Manage time in an efficient and effective manner to prioritize workload and meet client and organizational goals. Proactively and effectively communicate knowledge gained from education to others in the organization using presentations, emails, and conversations. Maintain a positive attitude and an exceptional level of communication with residents, staff, and supervisors. Other duties as assigned by the Director of the Health Recovery Homes The right candidate should posses the following: Bachelor's Degree in Social Work, Human Services, or relate field Minimum of 2 years of Case Management Experience A drive to serve the underserved with compassion and humanity A deep understanding of poverty Experience working with individuals experiencing homelessness, medical needs, mental health challenges, and/or substance use challenges. Possess strong documentation, organization, and communication skills Ability to work independently Be willing to break down barriers, accept no dead ends, and be a fierce advocate for our patients Be a collaborative member of the GFC team to ensure high quality, respectful healthcare is received by all of our patients Be a collaborative member of the GFC team to ensure the success and growth of GFC as an organization
Work Hours:
This person is will be working Monday through Friday, 20 hours per week.
Physical demands:
The physical demands described here are representative of those that must be met by an employee/member to successfully perform the essential functions of this job: This position will primarily be an indoor position, but there will be times when the position requires comfort working in outdoor environments. This position is very active and frequently requires standing, walking, bending, kneeling, and stooping. While performing the duties of this job, the employee/member is regularly required to communicate with individuals who talk and hear. The employee/member may occasionally lift and/or move items over 50 pounds. While performing the duties of this job, the employee/member is regularly required to observe and comprehend using vision abilities that include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.
