Position Summary:
Is responsible for providing a continuum of care for guests of Wheeler Mission through case management, involving comprehensive assessment, individualized goal planning, and coordination of services to support guest stability and recovery. This role works closely with guests participating in addiction recovery programming who may also experience homelessness, mental health concerns, trauma histories, and other barriers to long-term stability. Key duties of this position include case management, crisis intervention, recovery support, mentoring, discipleship, counseling, relapse prevention support, advocacy, care coordination, and facilitating groups when needed. Center for Women and Children Key Duties and Responsibilities Brief description of duties in order of priority Case Management for Guests General case management directed toward achieving stability for each guest including: Assessment and Identification of each guests' resources, talents, skills and interests. Develop a care plan with written goals and methods to help the guest achieve those goals. Monitor the effectiveness, appropriateness, and efficiency of the plan, modifying as needed. Advise guests on best practices for success and achievable goals. Identify resources and needs, lead crisis intervention, provide information, and referrals. Program Oversight:
Administer program elements as designed: Monitoring guest progress and engagement in recovery programming Supporting accountability and program expectations within a residential recovery environment Facilitating small groups, recovery-focused discussions, and life skills programming Assisting guests in finding a Foundation in Christ and supporting long-term recovery growth Coordination of Services:
Helps guest achieve goals through: Working collaboratively with other program staff members Collaborating with other agencies to coordinate services designed to create a network Advocating for guests when necessary Administrative and Other:
Ensure programmatic integrity through: Record keeping for guests through maintaining appropriate documentation of guest sessions for identifying important information, for agency referral and follow-up. Enter information in database that documents progress. Legal responsibilities include taking appropriate action according to the established protocol of the organization, maintaining guest confidentiality in all areas following biblical definitions of ethical conduct and behavior. Follows and ensure adherence to all facility polices. Participate in multidisciplinary guest case conferences that are designed to share information beneficial to the guest Self-care includes taking time to prepare for each day with study and prayer. Serves in unity with other staff members as well as practicing healthy work/life balances which includes participating in a faith community. Education/Experience:
2 years social service/ministry experience, specifically counseling, mentoring, case management, or recovery support At least 1 year of experience working specifically in addiction recovery or with individuals experiencing substance use disorders. Experience working with individuals experiencing mental health concerns, trauma, domestic violence, homelessness, or other barriers to stability and recovery preferred. Bachelor's degree in social work, counseling, psychology, human services, ministry, behavioral health, or related field required. Master's degree preferred. Required Knowledge Areas for Success:
Knowledgeable about community housing, recovery, behavioral health, and employment resources and able to help guests connect to appropriate services. Demonstrated ability to assist guests in obtaining appropriate documentation, utilize community resources, and support employment and housing stability. Best practices in case management, addiction recovery support, and faith-based concepts to serve guests with excellence. Trauma-Informed Care, crisis intervention, de-escalation, relapse prevention support, and mental health support. Ability to maintain appropriate boundaries, build rapport, and support accountability within a residential recovery environment. Technical/Software Knowledge:
Intermediate proficiency in Microsoft Office Suite, including email, Word and Excel Intermediate proficiency in Resident Database and basic ability in Paycom Mission Alignment:

To perform the essential job functions of this position, a candidate must be capable of performing many physical functions including: Walking short distances with the ability to ascend and descend ten flights of stairs Physical dexterity sufficient to type and operate standard office equipment Physical ability to perform normal office functions with the endurance to remain standing for long periods of time Strength to lift office supplies and equipment up to 20 pounds. Ability to drive a vehicle with multiple occupants in a city environment. Other Requirements for Employment This is a full-time 40-hour per week position, typically staffed throughout the week with some evening and weekend hours required. Must possess a valid state issued driver's license and have a driving record without major moving violations, including convictions for DUI, Reckless Driving, or Driving While Suspended. Must be insurable according to standards established by Wheeler Mission's risk management insurance agency.
