The Assertive Community Treatment (ACT) Case Coordinator provides comprehensive, community-based mental health services to individuals with severe and persistent mental illness. Working closely with a multidisciplinary ACT Team, you will help clients access the supports they need to improve stability, independence, and overall quality of life. Minimum of 2 years of experience in mental health case management or a related field. Experience working with individuals with severe and persistent mental illness preferred. Valid driver's license, auto insurance, and reliable vehicle Ability to work flexible hours, including evenings and weekends, as needed. CPR and CPI certification preferred.
