Job Description

MISSION STATEMENT
Swanson Center's compassionate staff is dedicated to improving lives by providing comprehensive, expert mental health services.
SUMMARY:
Case Managers perform a variety of clinical tasks such as conducting client intakes, participating in treatment and discharge planning, executing treatment plans, performing individual and group services, and ensuring that clients receive necessary follow-up care and assistance.
QUALIFICATIONS/DEGREE
Bachelor's degree in Social Work, Psychology, Addictionology, or a health related field with at least two years of post-degree experience working in behavioral health. Prior service in a community mental health center is preferred. Must possess a good driving record, valid driver's license, and a registered four-wheeled vehicle.

ESSENTIAL TASKS AND RESPONSIBILITIES
Visits clients at other facilities or in their homes. Demonstrates courtesy, professionalism, and commitment to the Center's mission at all times. Performs services necessary for the client's treatment, comfort, and convenience such as transportation, assistance with personal affairs, and help with seeking employment. When qualified and directed, serves as a member of the Emergency Crisis Team and participates in on-call duties. Performs standard case management tasks such as linkage, monitoring and follow-up, referral service planning, client advocacy, crisis intervention, intake activity, discharge planning, staffing, home visits, and other duties that conform to recognized treatment modalities. Complies with productivity standards described in the Center's collective bargaining agreement. Maintains accountability for assigned clients' progress, the continuity of their care, and the appropriate disposition of their cases. Fully ensures the timely, thorough, and accurate submission all required paper and electronic records of client care and staff activity. Attends and participates in designated staff meetings. Advocates for clients in order to improve their general well-being. Assists with the preparation of commitment materials, emergency detention procedures, and court referral proceedings. Depending on the program assigned, licenses residential homes and apartments. Provides supervision to residential householders in accordance with the Residential Rules and Regulations. Uses standard precautions to control the spread of infection and parasites. Develops and maintains linkages with other caregivers and the state hospital system. Complies with the Center's Code of Conduct, Code of Ethics, and other policies and procedures. Makes recommendations to supervisors regarding client treatment and services. Participates fully in the quality assurance systems of the Center. At all times, maintains a valid driver's license, observes traffic laws, and practices safe driving when transporting Center clients. Maintains a functional, registered vehicle for transporting clients.
