Job Description

Pay is commensurate with experience within a range of $20.00 to $27.86 per hour. Full time, non-exempt, benefits eligible. Responsibilities:
The primary responsibility of the Child & Family Services Case Manager will be case management services, resource acquisition, crisis intervention, and outreach activities to specified youth and their families. Specific duties include but not limited to: providing case management services, resource acquisition, crisis intervention, outreach activities and coordinate these services with other community health and social service agencies; delivering services in a variety of locations, including but not limited to the client's home, the school, the Child & Family Services program site, or another community setting, on an as-needed basis; providing transportation to and from various activities or appointments as indicated by the treatment plan or as instructed by the QMHP; providing services on an as-needed basis both with respect to time-of-day and location; delivering reimbursable hours of service compliant with established standards; responding to crises while on call in rotation with other similarly trained staff. To be successful in this position late evenings and/or weekend hours are required to meet the needs of our clients and their families regular and predictable attendance is an essential function of the job. additional responsibilities may be assigned as circumstances warrant.
Qualifications:
The Child & Family Services Case Manager position requires but not limited to: a BA/BS degree or four years of equivalent education and/or experience working in the human services field. reliable transportation and automobile insurance; and a valid, unrestricted driver's license; prior computer experience and basic computer fluency; must be able to achieve within one's first 6 months of employment a working competency in the management and use of the Center's electronic medical record (EMR) software as demonstrated by timely and accurate documentation of the clinical record; an ability to work independently and in cooperation with others; demonstrated strong interpersonal skills in working with diverse publics, consumers, and referral agents; flexibility in assigned working hours; an energetic and positive approach to the rapidly evolving changes and challenges of a complex workplace.
