Case Management is responsible for working with clients who have had a Brain Injury and helping them to identify deficits and goals and connecting them with resources to improve their quality of life.
Will be responsible for:
Meeting with client to identify goals and needs and create a plan of action Supervision of staff providing for their therapy needs - Cognitive, Behavioral, and Life Skills This position will perform client intakes, identify deficits and establish goals, communicate and work closely with staffing coordinator and perform supervision of all therapies provided on a routine basis to ensure client needs are met.
Must have:
Excellent communication and organizational skills Ability to work independently and with a high-needs population of clients The ability to adapt to ever-changing client needs Ability to work well with the Brain Injury Program manager and others on the disciplinary team. Requirements - Must have ONE of the following degrees: Master's Degree in a Behavioral Sciences field, such as psychology or social work Master's Degree in Special Education Benefits available including, medical and dental insurance, PTO, vacation time and a flexible schedule.
