Work Schedule:

Complete assessments and provide case management services to persons 60 years of age and older residing in Sedgwick, Butler and Harvey Counties requesting Older Americans Act (OAA), short-term In-Home and/or Nutrition services. Complete functional assessment and reassessments for consumers requesting service. Travel to consumers in a tri-county required. Interview consumers demonstrating active listening skills. Complete required forms and accurately data enter in required time frame. Submit accurate and complete documentation and billing. Provide Care Management to consumers. Identify individual needs Develop care plan implementation, and provide on-going monitoring of services and consumer needs. Educate consumers on community based care and resource options. Provide accurate and unbiased information or referrals as needed including pertinent materials. Schedule, complete assignments and documentation with in required deadline. Maintain and data entry of billing and all case notes in required time frame in the electronic client record Management Information System (MIS), Nterfuse. Maintain an up-to-date Microsoft Outlook calendar.
Minimum Qualifications:
Bachelor's degree from an accredited college or university in Social Work, Gerontology, Nursing, or Counseling. Minimum of one (1) year experience in the geriatric service field. Minimum of one (1) year of Case Management experience. Valid State of Kansas driver s license and proof of automobile insurance.
Preferred Qualifications:
Master's degree from an accredited college or university in Social Work, Gerontology, Nursing, or Counseling. Two (2) years' experience in the field of aging or an Area Agency on Aging. Ideal candidate will have experience with physical disabilities (PD), traumatic brain injuries (TBI), persons with a mental health diagnosis, and/or individuals with intellectual or developmental disabilities (ID/DD). Licensed Social Worker or Nurse. Bilingual skills a plus. Applicants have rights under Federal Employment Laws.
