I/DD Case Manager

POSITION SUMMARY
Case management positions consist of advocating and assisting participants with waiver services.
DUTIES AND RESPONSIBILITIES
Conduct annual assessment and person-centered service plan Conduct monthly face to face visits with participants Assist clients with accessing community resources Monitor participants progress with waiver services including health, safety and welfare in monthly contact note Assist with building a network of supports Assure participant choice and access to community resources. Return phone calls and emails promptly. Complete and submit documentation by required deadlines. Perform other related work as required and assigned.
QUALIFICATIONS
BA degree in human services: psychology, social work, sociology, special education or other related behavioral sciences. One year of experience working with ID/DD individuals (ex: volunteering or having a relationship with family or friend who may have ID/DD). OR one year of case management experience. Valid Driver's license and vehicle insurance required Strong organizational skills Ability to multi-task Excellent communication skills Be self-motivated Be able to conduct and facilitate meetings The ability to collaborate with other team members Ability to advocate for the needs of participant
JOB REQUIREMENTS / EXPECTATIONS
This job consists of sitting to complete necessary paperwork on computer Must be able to operate a motor vehicle and have reliable transportation with valid insurance Must provide verbal and communication with team members Job setting is at the corporate office and/or home office setting Documentation is time sensitive and must follow waiver regulations Work schedule is Monday-Friday 8am-5pm. Schedules can vary depending on participant visits. Flexible working hours.
