Job Description

Salary:
$35,000 - $39,375 annually Educational Requirements Bachelor's Degree in Human Services or a related field required Minimum of one year of post-graduate experience required Duties & Responsibilities Provide case management services to assigned consumers in accordance with individualized treatment and service plans Coordinate and monitor services to support consumers' behavioral health, medical, social, and community resource needs Maintain accurate, timely, and current documentation in compliance with agency, state, and federal requirements Collaborate with consumers, families, providers, and community partners to ensure continuity of care Assist consumers with accessing community resources, benefits, and supportive services Participate in treatment team meetings and contribute to care planning efforts Adhere to all Pathways policies, procedures, and confidentiality standards Perform additional duties as assigned within the scope of the position Skills & Knowledge Experience working in a behavioral health setting preferred Strong communication, organization, and interpersonal skills Ability to manage multiple tasks and maintain accurate documentation Competency in using computers, electronic health records, and other electronic devices required for job duties Ability to work independently while collaborating effectively within a multidisciplinary team
