Case Manager

Supervise the progress of each person with disabilities to in to include writing the Individual Support Plan (ISP) and Behavior Support Plans (BSP) documents, chairing ISP/BSP meetings, developing training and service objectives, and instructing staff on continuous implementation of ISP's and BSP's. Monitor and document process of clients to include data collection, periodic progress reports, and other documentation as stipulated by state governing bodies or by company management. Develop and supervise implementation of ISP's and closely monitor staff for correct implementation and effectiveness. Also responsible for determination of effectiveness.

Duties include completing quality assurance inspections, oversight and completion of monthly documentation and reports, and assisting with updating work schedules for Direct Support Professionals.
