Job Description

Ageless Healthcare has a Case manager position available! We are a personal care service (HCBS) company opening in Baton Rouge soon. Must be familiar with
LT-PCS, CCW, NOW, SIL WAIVER
& other programs. Be familiar with rules and regulations from Louisiana Department of Health. Be familiar with the plan of care, calculating budget sheets, and writing individual service plans. Must know Microsoft Office including Word & Excel. Must be on call 24/7. Pay negotiable upon Experience.
