Job Description
Position Summary The Case Manager is responsible for coordinating, monitoring, and documenting services for individuals enrolled in Louisiana Medicaid Home and Community-Based Services (HCBS) waiver programs. The Case Manager ensures services are delivered in accordance with Louisiana Department of Health (LDH) regulations, waiver standards, and person-centered planning requirements, while promoting individual choice, independence, health, and safety in the community. Essential Duties & Responsibilities Service Coordination & Person-Centered Planning Complete initial and ongoing assessments in compliance with LDH and waiver-specific requirements Develop, implement, and update Person-Centered Care Plans Ensure service plans reflect individual goals, preferences, strengths, and assessed needs Coordinate authorized waiver services and community resources Facilitate interdisciplinary team (IDT) meetings and service plan reviews as required Monitoring & Compliance Conduct required face-to-face visits, monthly contacts, and annual reassessments per Louisiana HCBS standards Monitor service delivery to ensure services are provided as authorized and documented Ensure compliance with OAAS and OCDD guidelines Identify, report, and follow up on incidents, complaints, abuse, neglect, or exploitation in accordance with LDH policy Participate in internal reviews, audits, and quality assurance activities Documentation & Reporting Maintain accurate, timely, and compliant documentation in the agency's electronic case management system Ensure all records meet Medicaid, LDH, and agency documentation standards Complete required reports, service authorizations, and case notes within established timeframes Advocacy & Support Advocate for individual rights, choice, dignity, and self-determination Educate individuals and families on HCBS waiver services, rights, and responsibilities Assist individuals in navigating healthcare, behavioral health, housing, and community supports Collaboration & Communication Maintain regular communication with providers, families, support coordinators, and LDH representatives Collaborate with supervisors and program staff to address service gaps or risks Participate in staff meetings, trainings, and professional development activities Qualifications Education Bachelor's degree in Social Work, Human Services, Psychology, Nursing, or related field Experience Two (2) years of experience in case management, support coordination, or HCBS services preferred Experience working with individuals with disabilities, aging populations, or chronic conditions preferred Knowledge, Skills & Abilities Knowledge of Louisiana Medicaid HCBS waivers and LDH regulations Familiarity with person-centered planning principles Strong organizational, documentation, and time management skills Ability to manage a caseload independently Effective verbal and written communication skills Additional Requirements Valid Louisiana driver's license and reliable transportation Ability to travel within assigned service area Successful completion of background checks and required clearances CPR/First Aid certification (or ability to obtain) Join us if you're passionate about empowering individuals through compassionate support and innovative solutions!
