Job Description

Job Overview We are seeking a dedicated and energetic Discharge Planner/Case Manager to join our dynamic healthcare team. In this vital role, you will coordinate patient discharges, facilitate seamless transitions of care, and ensure optimal post discharge services for individuals who are need ongoing substance use treatment. Your expertise will help improve patient outcomes, reduce readmissions, and promote efficient utilization of resources. This position offers an exciting opportunity to make a meaningful impact within a fast-paced, patient-centered environment. Responsibilities Collaborate with interdisciplinary teams including providers, nurses, social workers, and counselors to develop comprehensive discharge plans tailored to each patient's needs. Coordinate post care services such as Intensive Outpatient Programs, medication assisted treatment, primary care, housing, and community resources to ensure continuity of care. Manage medical records and documentation related to discharge planning and utilization management while maintaining compliance with HIPAA regulations. Facilitate diagnostic evaluations and inpatient or outpatient case management activities to optimize patient recovery trajectories. Requirements Familiarity with managed care principles, utilization management practices, and medical records documentation standards. Experience working within behavioral health or mental health counseling frameworks is highly desirable. Excellent communication skills for effective collaboration with multidisciplinary teams and external providers. Ability to navigate complex healthcare systems including outpatient clinics, inpatient units, and community-based services. Pay:
$15.00 - $20.00 per hour
