Discharge Planner/Case Manager

Job Overview We are seeking a dedicated and energetic Discharge Planner/Case Manager to join our dynamic healthcare team. Responsibilities Collaborate with interdisciplinary teams including providers, nurses, social workers, and counselors to develop comprehensive discharge plans tailored to each patient's needs. Coordinate post care services such as Intensive Outpatient Programs, medication assisted treatment, primary care, housing, and community resources to ensure continuity of care. Manage medical records and documentation related to discharge planning and utilization management while maintaining compliance with HIPAA regulations. Facilitate diagnostic evaluations and inpatient or outpatient case management activities to optimize patient recovery trajectories. Requirements Familiarity with managed care principles, utilization management practices, and medical records documentation standards. Experience working within behavioral health or mental health counseling frameworks is highly desirable. Excellent communication skills for effective collaboration with multidisciplinary teams and external providers. Ability to navigate complex healthcare systems including outpatient clinics, inpatient units, and community-based services.
