Position Overview:
Under the supervision of the Director of Housing Services, the Case Manager is responsible for assisting people experiencing homelessness in locating and sustaining safe, stable housing for a period up to two years. The Case Manager is a source of support and guidance to a caseload of 25 individuals. Case Managers are trained to assist survivors with identifying barriers to housing stability and engage in goal planning that links them to the resources and skills needed to achieve long-term stability. The primary goal of this program is to support survivors in exiting to self-sustained permanent housing.
Duties and Responsibilities:
Conduct client assessments regarding housing needs and location preference Assist clients as needed during their move into housing (inspections, lease signings, accessing donated furnishings, coordinating with volunteer movers, and coordinating transportation) Create a mutually trusting relationship with clients which may result in the client's desire to fully engage in case planning/support services Develop individualized service plan tailored to meet the needs of clients Support clients in securing and transitioning to self-sustained permanent housing, including helping them build income, stability, independent living skills, and community supports needed to retain housing after program exit Monitor physical/mental health of the client Provide case management/support services as requested by client, including, but not limited to money management, adult living skills, stress management, coping strategies, employment/education info etc. Conduct home visits monthly and/or as needed Attend appointments with clients as needed Complete organization/funder required reports and service documentation in a timely manner Complete client case notes in a timely manner using organization's internal documentation system Update and maintain client files Protect the confidentiality of all records Other related duties as assigned
Skills, Experience, Education Required:
Bachelor of Social Work degree or bachelor's degree in the human services field with experience serving victims of domestic violence (Master's degree preferred). Lived and/or peer experience can substitute professional experience Training or familiarity with trauma-informed care, safety planning, crisis response Must have a valid Louisiana Driver's License and reliable transportation to conduct home visits Strong advocacy skills and ability to collaborate with community partners, other service providers, family members and landlords Compassion, patience, supportive, demeanor with the ability to assist clients without judgment as clients may be affected by substance use, mental illness, sex work, institutionalization, health issues, and may be distrusting of service providers Travelers Aid Society of New Orleans is an Equal Opportunity Employer.
