Job Description

Who We Are United Cerebral Palsy of Metro Boston (UCP) is a progressive, non-profit agency committed to advancing the independence, productivity and full citizenship of people with disabilities. The administrative office is located in Watertown, MA but provides service throughout eastern Massachusetts. This is a great opportunity for those seeking a diverse Human Services position in a supportive team with an opportunity for growth. The Options for Community Living Program is a community-based supported living model of services designed to meet the needs of individuals with developmental and physical disabilities. Individuals receive supports while residing in accessible apartments in eastern Massachusetts communities. Each individual served is assigned a case coordinator who provides support addressing skill areas, such as personal care attendant management, transportation, finances, health management, household management, community access, self-advocacy, self-care and social/recreation. Case coordinators' tasks are limited to those which an individual cannot presently manage and are purposely geared toward enabling a person to live as independently as possible. There is strong emphasis on goal setting and decision-making. What You'd Do Meet with caseload regularly, providing assistance in areas determined by their individual support plan. Coordinate services, which may include: skills training, Personal Care Attendant/Homemaker services, peer counseling, therapy services, healthcare, financial management and transportation. Participate in the development of support plans, emphasizing the empowerment of the persons served. Assist with accessing community resources in the areas of healthcare, housing, employment, recreation, transportation, adult education, independent living skills training, Independent Living Center services, and others as needed. Supervise, support and teach skills in the areas of daily living, money management, personal care, household management, community involvement and relationship building. Assist with self-advocacy. Who You Are A detail-oriented, self-motivated, empathetic professional who is a resourceful problem solver and clear communicator. A strong ability to work independently is required. Two years experience in human services, healthcare or other direct service field is preferred. Having prior experience in independent living or supportive living services is a bonus but not required.
Working Conditions:
The Case Coordinator is expected to commit the time and talent necessary to fulfill the requirements of the position in an exemplary manner. This position is a combination of in-person visits with consumers in their own homes and remote administrative work plus team meetings. Must have reliable transportation to support in-person visits to individuals on caseload.
