Job Description

We are seeking a compassionate and organized Case Manager to support individuals and families enrolled in Adult Family Care (AFC), Temporary Assistance (TA), and Intensive Supported Community Housing (ISCH) programs. The Case Manager will coordinate services, connect clients with community resources, monitor progress, and help individuals achieve stability and independence. Position Summary The Case Manager provides care coordination and support services to clients participating in Adult Family Care (AFC), Temporary Assistance (TA), and Intensive Supported Community Housing (ISCH) programs in accordance with Massachusetts state regulations and agency policies. Essential Functions Complete assessments, service plans, and ongoing case documentation Conduct home and community visits in compliance with AFC and housing program requirements Coordinate healthcare, behavioral health, housing, and community-based services Maintain confidential records in accordance with HIPAA and Massachusetts privacy regulations Ensure timely documentation and compliance with MassHealth and program standards Participate in multidisciplinary meetings and case reviews Provide advocacy, crisis intervention, and referral services Minimum Qualifications Bachelor's degree in Social Work, Human Services, Psychology, or related field preferred Experience with MassHealth, AFC, housing services, or behavioral health preferred Knowledge of Massachusetts community resources and service systems preferred Strong written and verbal communication skills Ability to travel within Massachusetts as required Physical & Work Requirements Ability to travel to client homes and community locations Ability to use computers and electronic documentation systems Occasional evening or flexible hours may be required Background Requirements Employment is contingent upon successful completion of: CORI (Criminal Offender Record Information) check Reference checks Verification of education and employment eligibility Compliance This position complies with applicable Massachusetts labor laws, HIPAA regulations, and program-specific requirements for AFC, TA, and ISCH services.
Pay:
$22.00 - $25.00 per hour
