Community Connections Inc. is looking for Case Managers to join our Day Center team based in Fairhaven, MA. In this role, you will support individuals with disabilities by coordinating all services for your assigned caseload. You will partner with Community Connections in their mission to provide optimal independence and enriching services to people with disabilities. Case Manager:
Be responsible for Consumer Case Management Coordinate Day Habilitation Service Plans Work in conjunction with Developmental Specialists in developing and documenting all DHSP goals and objectives Develop and maintain all case management records for assigned caseload Assist individuals physically as necessary Ensure and safeguard individuals' human dignity, rights, and liberties What You'll Need as aCase Manager:
High School Diploma or General Education Degree (GED) Bachelor's degree in relevant human service field preferred 3+ year's experience and/or training working with individuals with disabilities Valid driver's license and satisfactory driving record Proven problem solving abilities Excellent written and verbal communication and interpersonal skills Demonstrated ability to work independently Strong computer and word processing skills Who You Are as aCase Manager:
