Job Description

Employment Case Manager Waystone is currently looking for a dedicated Employment Case manager based out of our Day Habilitation Program in Haverhill. As an Employment Case Manager, you will coordinate personalized support plans, assist individuals in building meaningful careers, and connect individuals with inclusive opportunities- ranging from paid employment and volunteer work to skill-building activities. Your work centers on promoting independence, dignity, and belonging, while cultivating strong relationships with employers, families, and community partners. Haverhill, MA (Day Habilitation Program)
Schedule:
Monday - Friday 8:00am - 4:00pm. This position is benefit eligible as of date of hire.
Compensation:
This is a 40 hour non-exempt, full-time position. The hourly rate is $23.00. You will be a union member to SEIU Local 509. Why join the Waystone Team? Job Responsibilities:
Coordinate individualized services by working closely with the Director of Community Learning to coordinate and document all aspects of service delivery, including case conferences, support plan meetings, and progress reviews. Implement support plans and ensure all services—paid work, volunteer opportunities, community activities, and skill-building—align with each individual ISP, DHSP, Career Plan, and Behavior Support/Treatment guidelines. Provide direct support and deliver respectful personal care and assistance tailored to each person's medical, physical, and psychological needs. Implement crisis plans and medical protocols accurately. Maintain accurate documentation by completing all required records (attendance, progress notes, assessments) in a timely and compliant manner using HCSIS and other systems. Foster community relationships by build strong, professional relationships with employers, families, funding sources, and regulatory agencies. Promote natural support and reduce reliance on formal services.
Additional Responsibilities:
Support career development by assisting with assessments, career plans, and job coaching. Conduct job site check-ins, employer communications, and facilitate small group sessions and community outings. Respect and protect the legal, civic, and human rights of all individuals served. Maintain strict confidentiality of personal information. Stay current with all mandatory certifications (CPR, First Aid, etc.) and participate in ongoing training initiatives from Waystone and state agencies. Transport individuals as needed to support their participation in community and vocational activities. Assist in keeping all environments safe and clean by promptly reporting any issues to the appropriate personnel.
