Job Description

Schedule:
Full-Time | Typically 12:00 PM - 8:30 PM with some scheduling flexibility
Setting:
Partial Hospitalization Program (PHP) & Intensive Outpatient Program (IOP) Grand Rising Behavioral Health is seeking a highly organized and compassionate Case Manager to support clients enrolled in our Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP). This role is ideal for someone who enjoys working directly with clients while also managing administrative and coordination responsibilities that help ensure a smooth treatment experience. The Case Manager plays a key role in supporting client needs throughout the day, coordinating resources, assisting with aftercare planning, and helping clients navigate insurance, financial, and employment-related matters that support treatment engagement. This position works closely with clinicians, medical providers, and administrative staff to ensure clients receive comprehensive and coordinated care. Key Responsibilities Client Support & Care Coordination Serve as a primary point of contact for client support needs during program hours Assist clients with care coordination and case management needs while in treatment Support clients in accessing community resources, outpatient providers, and recovery supports Assist clinicians with discharge planning and aftercare coordination Conduct outreach to referral sources, outpatient providers, and community partners as needed Provide guidance and support to clients navigating housing, employment, and recovery resources Administrative & Program Support Assist with daily program operations and help maintain a supportive therapeutic environment Provide administrative support related to client care coordination Help ensure client documentation, forms, and program requirements are completed Assist with insurance benefit education and coordination with the admissions/billing team Support clients in understanding financial responsibilities and payment plan options Client Advocacy & Benefits Support Assist clients with FMLA, short-term disability, and workplace documentation to support treatment participation Help clients coordinate with employers, HR departments, and other external systems when appropriate Support clients in navigating insurance benefits and treatment authorizations Advocate for client needs to help reduce barriers to treatment engagement and program completion Team Collaboration Work collaboratively with therapists, psychiatric providers, and administrative staff Participate in treatment team meetings to communicate client needs and coordination updates Support a trauma-informed and recovery-oriented treatment environment Qualifications Bachelor's Degree required in Social Work, Psychology, Human Services, Public Health, or a related field Experience working in behavioral health, mental health, substance use treatment, or healthcare settings preferred Strong organizational and administrative skills with the ability to manage multiple client needs simultaneously Experience with care coordination, case management, or client advocacy preferred Familiarity with insurance benefits, FMLA, or disability paperwork is a plus Strong communication and interpersonal skills Ability to work effectively in a fast-paced, team-based treatment environment Comfortable working directly with individuals experiencing mental health and/or substance use challenges Benefits & Professional Support Grand Rising Behavioral Health is committed to supporting our team members both professionally and personally.
