Job Description

Program Case Manager Job Summary:
Provides supervision, opportunities, and skill development to persons served by the Northeast Arc in their community-based day and work supports program. Pay $24.00-$25.00 Monday through Friday 8:00am-4:00pm-
Full Time Major Responsibilities:
Provides oversight to assigned caseload by attending scheduled meetings, maintaining regular and professional contact with the participant's team, completing all required documentation, and ensuring the participant's progress in the program as determined by his or her goals. Creates individual goals to reflect the participant's vision regarding skill development. Maintains all required documentation within required timeframes to include: Daily Data Monthly/Quarterly Progress Notes/6-month summaries in HCSIS Interest Inventories Assessments Confidential Record updates Makes sound clinical decisions and appropriate interventions but seeks assistance from a supervisor when necessary. Provides transportation to persons served in personal vehicle or agency vehicle when one is available. Assists in obtaining appropriate volunteer placements for persons served and provides ongoing coaching support in these placements to ensure continued success. Meets the physical and personal care needs of the people supported by the Program. Follows HIPPA, security regulations and / or guidelines and maintain confidentiality at all times.
Education and Experience:
High School Diploma or equivalent required. 1 year experience working with intellectually and/or developmentally disabled adults or other related experiences preferred.
Skills, Knowledge and Abilities :
Excellent organizational and interpersonal skills. Effective communicator in both person and writing. Team player, flexible, and able to adapt to a changing schedule. Ability to communicate in English and/or ASL via direct communication. Must have working knowledge of Microsoft Office products and associated applications. Valid driver's license required.
Physical Demands:
Must be able to stand/walk up to one hour uninterrupted.
Work Environment:
Environment consists of program areas, volunteer sites, vehicles and various locations out in the community. Town of Stoneham, MA
