We provide culturally sensitive addiction recovery services, healthcare access, and educational support. Position Overview:
The Case Manager will work with residents in the transitional sober housing program, ensuring they have the support they need to continue their recovery journey. This role involves individual sessions, group facilitation, and creating personalized treatment plans. Case Managers also act as liaisons between residents, clinical team, medical providers, and community resources. This position aims to support residents in their continued reintegration into the community, assisting with job placement, educational goals, and family involvement. Key Duties and Responsibilities
  • Develop personalized recovery and life goals with residents, focusing on employment, education, and personal development
  • Meet with residents weekly to assess progress, offer guidance, and modify care plans as necessary
  • Lead group sessions on recovery, relapse prevention, life skills, and emotional well-being
  • Coordinate aftercare resources, including referrals to outpatient services, sober living programs, and family reintegration
  • Connect residents with local support services such as employment opportunities, housing assistance, healthcare, legal aid, and financial support programs
  • Respond to resident crises, providing emotional support, de-escalation, and intervention services
  • Complete and maintain accurate records, including resident progress notes, treatment progress, and resource referrals, by the end of each week
  • Act as an advocate for residents in accessing external services, negotiating with service providers, and ensuring residents' needs are met
  • Assist in monitoring medication administration, ensuring compliance with recovery plans, and managing resident responsibilities such as household chores and curfews
  • Provide coaching on essential life skills such as time management, budgeting, communication, and job search strategies
  • Facilitate family involvement and the rebuilding of personal support systems as part of each resident's recovery journey
  • Assist residents in resume building, job searches, interview preparation, and career development planning
  • Assistresidents in participating in daily household and community-building activities to foster independence and personal growth.
  • Participate in regular team case reviews to assess resident progress, address challenges, and adjust interventions as needed
  • Ensure residents adhere to program rules, and address non-compliance through interventions, behavioral contracts, or escalations as necessary
  • Help residents navigate housing applications and maintain stable living arrangements after completing the program
Qualifications Bilingual/Bicultural Preferred:
Strong written and verbal Spanish language skills Bachelor's degree or certifications in social work, Psychology, Counseling, or a related field is preferred; an equivalent combination of education and life experience will be considered At least 1-2 years of experience in case management, preferably in a substance use disorder or mental health setting Understanding of substance use disorder recovery models, including MAR, harm reduction, and trauma-informed care Ability to work effectively with diverse populations, particularly the Latino community Strong ability to manage crises and de-escalate potentially challenging situations Excellent interpersonal, verbal, and written communication skills Strong time management and multitasking abilities, with attention to detail in documentation and case notes Ability to work collaboratively with clinical teams, medical staff, and residents to ensure holistic care Proactive and solutions-oriented, with the ability to make quick decisions and offer creative solutions Knowledge of local resources, including healthcare, legal aid, housing assistance, and employment services CPR/First Aid certification is preferred Valid Driver's License (Required) Willingness to work rotating shifts, including some evenings and weekends as needed High level of professional integrity, maintaining confidentiality and adhering to ethical standards The Latin American Health Alliance is an equal opportunity employer.
